Is your school still using paper discipline referrals? If so, you're likely dealing with missing [music] paperwork, inconsistent reports, and poor visibility. In this video, I'll show you how to create an online discipline referral form with Jotform. >> [music] >> Referrals will be easy to collect and easy to track. So, let's take a look at how to set this up. I'm on the My [music] Workspace page in my Jotform account. You can open your own Jotform account for free >> [music] >> at jotform.com. Once you're there, find the create button. It's on the top [music] left portion of the screen. Click that, and then select form. You [music] can start from scratch and use Jotform's drag-and-drop form builder, use a customizable form [music] template, and more. I'm going to use Jotform AI. I'll simply tell it what I need, and it will create a customizable form [music] in seconds. You can use voice commands or use your keyboard to enter a prompt. I have a prompt prepared, so I'm going to enter it >> [music] >> and hit the send button. Now, the more detail you provide here, the better. Ask for drop-down menus for behavior types, and make key fields required for consistency. Use multiple-choice questions so teachers [music] can submit a referral quickly. As you see, our form is ready to go. Uh that only took a few seconds, [music] and uh Jotform AI has a form ready for us to customize. You can use the form builder [music] to add a school logo, text, images, and more. You can access the form elements on the left side of the screen here, and you just drag and drop whatever form element you want on there. You can remove it just as easily. Or you can continue working with Jotform AI via the form copilot on the right side of the screen. When the form is ready to go, sharing it [music] is easy, and you have lots of options. To view your options, click publish in the form builder. It's right up top to the right of the settings. In the quick share menu, you can send [music] a direct link, share via mass email, or generate a QR code for the staff room. There are more options on the menu to the left. Options that could be a great fit for this form include [music] the embed option. You can copy and paste this code into the staff section of your website to share this form. And you have platforms. These are third-party publish options including Canva and WordPress. And then you have PDF. You can actually download a fillable version of your form in PDF format. So like I said, there are a lot of options, so pick one or more that work best for you. Every single submission is automatically stored in Jotform Tables, so you can view [music] and sort your discipline referrals. To access your submissions, go back [music] to your workspace. Find your form. Click the more option on the right. And then click submissions [music] in the data column. You can view all referrals in a single dashboard. Filter by student or date and spot patterns instantly. You can even ask Jotform AI to summarize your data and more. Now your data isn't just collected, it's usable.
