The Velocity Operating Model requires three conditions to hold simultaneously: decision flow, signal integrity, and cadence. The first two conditions have instruments that read them continuously — the decision aging record reads whether questions are moving, the epistemic integrity instruments read whether the picture is accurate. The third condition has no equivalent instrument in most governance architectures. Cadence is either assumed to be present because meetings are scheduled, or it is treated as a calendar problem to be solved by increasing meeting frequency.
